You must inform us of the intention to make the donation filling inthe following form. The following information needs to be included:

• First and last name
• Nat’l ID no
• Full Address
• Amount of the donation

We will send you, via email, a donation application with your identifying information.

Once you receive it, you can deposit the amount into the account of «Fundación para la Investigación Biomédica del Hospital Universitario La Paz», ES89 2038 1050 5160 0078 8994, either through an electronic transfer or by visiting a local bank branch. The payment reference should say «Donación_LastName» (where LastName is replaced with your last name).

Once the amount has been received, the Foundation will send you within 30 days the definitive Donation Certificate that will allow you to deduct the amount according to tax law.
